CIHR Gold Leaf Prizes

This awarded to individuals or teams whose research findings are unique, innovative and represent a ground-breaking discovery in any pillar of health research that has been seen to significantly influence knowledge in the field.

Nominations

• The online nomination form
• Three attachments that substantiate the impact of the discovery. This may include, as appropriate, letters of support, and/or a list of research contributions and activities arising from the discovery, outcomes, publications and publication impact, conference presentations, and other evidence (two pages for each attachment)
• The  Applicant Consent form for Use and Disclosure of Personal Information
• The Consent to Disclosure of Personal Information
• One electronic copy of the full nomination package on USB key, submitted in a single sealed envelope showing the complete return address of the sender

This prize will be awarded to an investigator at the beginning of their career who shows great potential and is expected to continue to produce research of exceptional merit. Nominated candidates must hold a full time research appointment (e.g., faculty appointment providing eligibility to apply for grants and/or supervise trainees) for a period of 0 to 10 years.

This may include, as appropriate, letters of support, and/or a list of research contributions and activities, outcomes, publications and publication impact, conference presentations, graduate students supported or trained, and other evidence (two pages for each attachment)
• Canadian Common CV
• A five-year research plan: The nominee must include a research plan for the next 5 years detailing the expected research goals and rationale, an explanation of how the goals are likely to be achieved, a rationalization as to where the research will be carried out and in collaboration with whom, and details on the timeframe (5 pages maximum)
• The  Applicant Consent form for Use and Disclosure of Personal Information
• The Consent to Disclosure of Personal Information
• One electronic copy of the full nomination package on USB key, submitted in a single sealed envelope showing the complete return address of the sender. Be advised that USB keys will not be returned to the nominators

In 2016, the prize will be awarded for outstanding leadership in patient engagement. CIHR believes strongly in the importance of making patient engagement part of the research process. This prize will be awarded to an individual or team that collaborates with patients, advances patient engagement as a priority and focuses on outcomes that are important to patients.

Nomination

The recipient(s) of the CIHR Gold Leaf Prize for Transformation: Patient Engagement will be required to give a keynote talk at a future Strategy for Patient-Oriented Research Summit (held once every 18 months in Ottawa). Details regarding the event will be communicated to the prize winner(s) following notification of the prize.

Nominations must include the following items:

• The online nomination form
• Three attachments that substantiate the impact of the nominee's leadership in patient engagement. This may include, as appropriate, letters of support (distinct from the patient and researcher statements outlined below) and/or a list of research contributions and activities, outcomes, publications, conference presentations, and other evidence (two pages for each attachment).
• Patient Statements: Patients engaged as partners in the research will provide statements describing their experience of being a partner, their roles in the research, and their understanding of the impact of the research. This can take the form of distinct statements from individual patient partners (minimum of two distinct statements) or a collective statement on behalf of all patients that were partners in the research process. (Maximum four pages for the entire attachment)
• Researcher Statement: Researcher(s) that have worked with the nominee will provide a statement about the impact of engaging patients in the research process. (Maximum four pages for the entire attachment)
