Community Partners
SOAR: Pathway to Wellness is a Quality Improvement Program partnering with four Indigenous communities and their organizational healthcare partners to improve care for people living with diabetes.
The name pays tribute to the four communities: Sheshatshiu Innu First Nation, Oneida Nation of the Thames, Abegweit First Nation, and Serpent River First Nation. The name and logo for the program were developed by the communities.
