Vehicle Rental Registration Form

Prior to booking a vehicle rental, please complete an information form located on a secure web page - Vehicle Rental Registration Form. The information in the form will be sent to the Corporate Insurance Office to ensure you are covered under the University’s Non-Owned Vehicle Insurance Policy.  Please note that a Western user name and password will be required to access this online form.
