Annual Report and Review

The purpose of the Annual Review is to provide an annual assessment of performance that allows recognition of a Member’s achievements, to provide feedback on plans for the coming year, and to identify areas for development in the Member’s Professional Practice, Academic Activity and/or Service activities, as appropriate to the Member’s Responsibilities and Workload.

Please note: The Annual Report and Review Form is fillable and your web browser may not be able to display them. To use these forms:

    1. Right-click on the link.
    2. In the menu that appears, click on "Save link as..." and save the file to your computer.
    3. Open up Adobe Reader (or similar PDF reader).
    4. Within Adobe Reader, click on the "File" menu on the top and choose "Open".
    5. Locate the file you just saved, click on it to choose it, and then click on the "Open" button.
