Read the Official Policy on Academic Absence due to Illness

Secondly, review OWL and/or the course outline to look for any comments from your instructor regarding academic consideration procedures. Inform your instructor or course/lab coordinator (OWL or your course outline may indicate who to contact) immediately about the course component you will not be able to attend or complete.

Is your course work you have missed/will be missing worth UNDER 10% of your final grade?
If so, please see your instructor for approval.

If your course outline says to submit a formal request to Academic Advising OR your missed/missing work is 10% or more of your final grade:
  1. Submit a ticket through our Academic Advising Help Portal
  2. Upload supporting documentation (required):**PDF or JPEG preferred
    • Completed  Student Medical Certificate . ( Emergency Department, Urgent Care notes may be accepted if the information is sufficient enough - duration of illness, impact on academics, and the severity of illness.)

*Incomplete submissions will cause delays!*

Please note:

If you can’t get an appointment with your family doctor when you are ill, it is expected that you will use a walk-in clinic or a virtual walk-in clinic or try to get into Student Health Services on campus. You must be seen at the time of illness for your notes to be valid.

Medical notes will be required for COVID. We are NOT accepting positive COVID tests as documentation. If an in-person clinic or your family doctor won’t see you then you must use on online health care provider or my virtual doctor https://www.mystudentplan.ca/purplecare/en/myvirtualdoctor

If you write your tests/exams with Accommodated Exams (through Student Accessibility Services) you will also need to fill out the Makeup Request Form through the Examination Services Portal. It is important you contact them separately from this academic absence approval process – and as soon as you can.
